Association between Cytomegalovirus (CMV) and Chronic Lung Allograft Dysfunction (CLAD) in Lung Transplant Recipients

چکیده

PurposeBronchiolitis obliterans syndrome (BOS), restrictive allograft (RAS), and mixed are major phenotypes of CLAD. In previous, older studies, CMV viremia has been associated with reduced survival. We hypothesized that, in a more modern era prophylaxis, would remain death, but also represent risk factor for CLAD specifically RAS/mixed phenotype.MethodsThis is retrospective cohort study all consecutive adult, first, bilateral-/single-lung transplants performed 2010-2016 who had ≥4 pulmonary function tests ≥1 CMV-PCR measurement. levels were divided into 3 categories: undetectable, <1000 IU/mL, ≥1000 IU/mL. Risks CLAD, RAS/mixed, or death/re-transplantation assessed by Cox proportional hazards models using as time-dependent categorical variable, adjusting variables listed table.ResultsOf 645 patients, 257 developed (143 BOS, 44 66 undefined/unclassified) 388 CLAD-free at last follow-up. univariate analysis, CMV-PCR≥1000 IU/mL was significant (HR 1.33, p=0.05). multivariate not whereas serostatus mismatch (Table). Cytomegalovirus Viremia in Lung Transplant Recipients Receiving

Background: Cytomegalovirus (CMV) disease is an important cause of organ transplant-related morbidity and mortality. During the last 5 years at our institution, prophylactic ganciclovir and hyperimmune globulin have been routinely administered to lung transplant recipients whenever the donor or the recipient was CMV antibody-positive. We sought to assess the efficacy of prophylaxis on viremia, ...

HLA-E⁎01:03 Allele in Lung Transplant Recipients Correlates with Higher Chronic Lung Allograft Dysfunction Occurrence

Lung transplantation (LTx) is a valid therapeutic option for selected patients with end-stage lung disease. HLA-E seems to play a major role in the immune response to different viral infections and to affect transplantation outcome, in Hematopoietic Stem Cell Transplantation, for example. Two nonsynonymous alleles, HLA-E(⁎)01:01 and HLA-E(⁎)01:03, have functional differences, involving relative...

Persistent cytomegalovirus-specific memory responses in the lung allograft and blood following primary infection in lung transplant recipients.

Primary CMV infection in lung transplant recipients (LTRs) is associated with increased mortality. We studied 22 donor CMV-positive, recipient-negative (D(+)R(-)) LTRs for the development of posttransplant CMV-specific immunity. We found that 13 of 22 D(+)R(-) LTRs (59.1%) seroconverted (CMV IgG Ab(+)). Using pooled peptides of the immunodominant CMV Ags pp65 and IE1, we detected CMV-specific C...

Cytomegalovirus infections in heart and heart-lung transplant recipients.

Of the first 250 heart and 41 heart-lung transplant recipients in the Papworth series, 244 who survived for longer than one month after transplantation were studied for evidence of cytomegalovirus (CMV) infection by serological methods. Overall, 43 (17.6%) patients acquired primary CMV infection and 76 (31.1%) had evidence of CMV reactivation or reinfection. The source of CMV infection was foun...
